What should the Certificate Identifying Exhibits or Documents look like?

An exhibit annexed to a divorce petition (Single Petition or Joint Petition), affidavit in support of petition (Single Petition or Joint Petition) or statement as to arrangements for children (Single Petition or Joint Petition) must be identified by a certificate of the Commissioner of Oaths before whom the affidavit of support or verification is sworn.

This is a cover sheet for documents used as exhibits to support facts stated in a divorce petition, affidavit or statement.

For High Court in Kuala Lumpur, you need to state the division of the court. Insert (FAMILY COURT, CIVIL DIVISION) one line below the county of court.

Leave the Divorce Petition Number blank. When you file your divorce petition, you will be assigned a case number, which must be marked on all subsequently filed documents.
